Tips for filling out the DocuSign

Your EF number will be in the welcome email you received after signing your Independent Contractor Agreement. If you cannot find your EF number, reach out to your manager

Fill out each box with the correct information. N/A will not be accepted

Text your manager as soon as you have completed the DocuSign and they will review and send it in for processing, ONLY your manager can send this in for processing. DO NOT email the DocuSign to Equis or our onboarding specialist
Documents for your contracting profile
1. Copy of your insurance license - If you do not have this, you can print your certificate from the website for your state's Department of insurance
2. Copy of your driver's license
3. Voided check, bank letter or direct deposit form

Voided Checks: The “check number” must be #101 or higher. (#100 will not be accepted). Starter checks will not be accepted. Account holder information must be pre-printed on check

Bank Letter - Must be typed and must be on bank letterhead. Letter to include: Account Holder Name, Routing Number and Account Number. Must be signed by bank representative.

Direct Deposit Form - Must have voided check on the form. Must be pre-printed. It cannot be hand-written. Bank logo must be on form. Form must include Account Holder Name, Routing Number and Account Number.
4. REG-ED Anti-Money Laundering (AML) certificate (REQUIRED *NO EXCEPTIONS*)

RegED is the ONLY accepted AML training at Equis Financial even if another certificate has been obtained recently.

Select Opt out of CE credits option, this AML Course cannot be applied towards CE credits. Upon completing the course take a screenshot and upload to your Contracting Profile.

5. Errors & Omissions Insurance (E&O)

E&O is a professional liability policy and is required by some of our insurance carriers. Coverage must at least be $1 million per claim / $2 million annual aggregate

Newly licensed agents should pay $59.17 to get coverage activated and then $34.17 monthly starting 30 days after initial payment. **Varies state to state**
6. Explanation letter for any "yes" answers (if applicable)

An example explanation letter is included in the DocuSign for you to use as a template. This letter must be typed - handwritten letters will NOT be accepted
After your manager has confirmed your contracting packet is complete, they will send it to the onboarding specialist for processing.

You will receive several emails from SureLC/SuranceBay to complete carrier contracting.

Special request to contract with our Premier Carrier - Government Personnel Mutual (GPM)

Work with your manager to see if you are eligible to request contracting with GPM. If you are eligible, please follow the instructions below:

Log in to your Equis Dashboard. Click GPM dashboard on left hand menu. Click GPM life contracting orientation. Then click start. If the GPM dashboard is not showing up on your Equis dasboard, use this link: https://www.equisfinancialtraining.com/challenge-page/gpm-orientation

The orientation should take about 30 minutes. You will receive a certificate that looks like this:

Email the certificate to your manager and they will request that you are contracted with GPM.
-The invitation to contract typically goes to spam, so be sure to check ALL inboxes.
